Registration, Change of Address and Withdrawals

Need to update your address?

To update/verify your home address, please log into your Parent Portal account and access the Guardian Update tile, then click on (change) next to your current address. The Address Change form will require recent Verification of Residency documentation (only required if you live in the district) and your photo ID before the request can be submitted.

Withdrawing a student from Prairie Middle School?

Please send an e-mail to PMSrecords@cherrycreekschools.org. You will be emailed a Withdrawal form which will require your students' full name, date of birth, grade just completed at Prairie, your new address and your new school. Prairies' Registrar will assist you. 

Registering a student at Prairie Middle School?
1. School Assignment by Address

Your neighborhood school is assigned at the Cherry Creek School District Admissions Office, according to the location of your residence and the school attendance boundaries. B​e aware that boundaries may change, so please contact District Admissions to confirm the assigned school for your address. 

2. CCSD Admissions Office

If your student is new to our school district, or returning to Prairie from another school, the first step is to enroll your student(s) with our District's Admissions Office. In addition to enrolling students to the district, the Admissions Office processes student records requests for former students (including unofficial transcripts). For additional assistance, or access to a computer, District Admissions has two offices listed below.

Required Documents for Enrollment:

  • A copy of your student's birth certificate (or alternative listed here)
  • Current immunization records · Proof of residency (examples listed here)
  • A government issued photo ID

3. Online Parent Forms

After enrolling with the District, you will need to complete the Parent Forms before class registration can occur. If enrolling for a school year that has not yet started, the Parent Forms will become available in July.

Access to the Parent Forms will be provided through an email sent to the parent/guardian listed as the first contact on the enrollment request and can only be completed by one parent/guardian.

4. Check-In at Prairie Middle School

You will receive an email setting up a registration appointment with Prairie Middle School on the designated grade level day:

Tuesdays - 6th Grade     |     Wednesdays: 7th Grade     | Thursdays: 8th Grade

*Note: If you have multiple students new to Prairie Middle School, you may bring them on the same day for their registration appointment

Please bring with you the following items:

  • Immunization Records
  • A copy of your student's IEP or 504, if applicable
  • Other pertinent school materials

Registration is approximately 1 hour and then, right after the registration appointment, your student will start their 1st day of school. Placement testing will take place at registration.
